* Attendee registration fees include admission to all plenary and educational sessions, networking with industry professionals, comprehensive conference materials, continental breakfasts, luncheons, hosted cocktail parties, awards dinner and complimentary self-parking.
** Spouse/guest registration fees include admission to all spouse/guest hospitality breakfasts, Monday and Tuesday night receptions and dinners, and farewell luncheon on Wednesday. Spouses and guests must register.

Conference Cancellation Policy: Refunds of registration/golf/tour fees, less a $79.00 processing fee per registrant, will be granted to cancellations received in writing by October 12, 2012. NO REFUNDS will be granted after this date. Substitutions may be made any time by contacting the CURT office at 513-563-4131.
